Ongoing Key Worker Sessions

The structure of key working in Coolmine House utilises the care and case management model and incorporates the community reinforcement approach and cognitive behavioural coping skill and tools which are incorporated into the overall model of the therapeutic community.

The sessions with the keyworker focus on desired outcomes in relation to care plans. A standard 15 item care plan is given to all clients but can be adapted and altered as appropriate. Reaching a goal as part of a care plan is measured by setting deadlines. 

All Clients who avail of the DFDP and Aftercare receive one-to-one key working sessions every two weeks. All clients on the Welcome programme receive key working sessions every week.
